Resolve a holdings break with a reason (a securities checker)

POST/ledger/fedwire-securities/holdings-breaks/{id}/resolution

The guard admits staff holding ledger:securities:write and ledger:securities:approve; the store requires a step-up within five minutes. The same resolution by the same checker replays; another is 409 conflict.
